Please note the hospital reserves the right to close the competition early should a substantial number of applications be received. Position SummaryThe Bone Health Clinical Nurse Manager will be a key member of the Multidisciplinary Team within St Joseph's Hospital Campus.
The purpose of this Clinical Nurse Manager (Bone Health) post is to coordinate, manage and lead the development and delivery of the Bone Health service within the St Joseph's Hospital Campus including undertaking the running of the DXA Scanning service and conducting nurse-led clinics.
The post holder will provide specialist nursing advice and support to patients (in-patient and out-patient) and their carer(s), throughout the care pathway.
The post holder will utilize skills of clinical expertise, education and management to ensure a seamless service for patients, carers and staff.
S/he will work closely with Bone Health Consultants, FLS (Fracture Liaison Service) ANP and other members of the relevant multidisciplinary teams in the hospital and community to ensure a coordinated, quality service.
S/he will be actively involved in education, audit and research.
The post holder will engage and lead the DXA unit and assess, screen, intervene, monitor and follow up (where required) the patients within the service including running a nurse-led DXA scanning service.
ResponsibilitiesGoals and Objectives:To provide a DXA service for inpatients and outpatients within the local catchment area of Beaumont and St. Joseph's Hospital and for local GPs.To provide a bone health and falls/fracture risk assessment for patients referred for DXA, in order to significantly reduce their future fracture risk.To contribute to the hospital's FLS service by capturing recent fragility fracture patients, ensuring rapid, yet comprehensive falls and bone health/fracture risk assessment.To educate, provide lifestyle advice, assessment and treatment plans to all patients, enabling them to self-manage their fracture risk positively.To reduce demand on hospital services from future acute fracture presentations by screening and treating those at highest fracture risk – i.e.
those who have had a first fracture (secondary prevention).To promote primary fracture prevention in those at next highest risk in our region – e.g.
fallers, patients with chronic diseases putting them at higher fracture risk through education, training and liaison with their health care providers on the necessity of addressing fracture risk.Clinical PracticeThe Clinical Nurse Manager will:
Lead and develop a nurse-led DXA bone density service.Provide 'one-stop' clinic assessments alongside, which will address, treat and monitor fracture and falls risk in patients at risk of same.Accept appropriate referrals from MDT colleagues.Assess, plan, implement and evaluate care and treatment using their specialist clinical knowledge and clinical skills.Undertake comprehensive patient assessment of patients at an early stage to prevent the major complications of osteoporosis (fractures) to include physical, psychological, social elements of care using best evidence based practice.Screen patients' bone density with the use of a DXA scanner to detect early bone density loss and skeletal changes.
The Clinical Nurse Manager for bone health will triage and schedule appointments.
Following completion of the scan the images will be analyzed by the Clinical Nurse Manager and DXA reports are closed and reviewed from the PACS system by the Radiology team.Begin treatment as early as possible to prevent additional disease manifestations i.e.
advising on osteoporosis/osteopenia drug treatment where indicated.
Co-ordinate investigations, treatment therapies and patient follow-up in conjunction with the bone health team.Monitor patients through telephone follow-up clinics at 4/12 and 12/12 to inquire regarding adherence to osteoporosis medications in those patients who have been through the FLS and initiated on osteoporosis treatment, in collaboration with GPs, community pharmacists and bone health team.Collect data and conduct data entry of clinical and radiological assessments for the purpose of evaluation and the future development of the service.Manage patient care to ensure the highest professional standards using an evidence based, care planning approach.Ensure continuity of optimal care for inpatients and outpatients.Act as an effective and professional role model.Ensure that policies, protocols and guidelines for patients receiving treatment in the bone health clinic are developed, implemented, audited and results disseminated.Engage in research where applicable.Attend meetings and provide feedback and support staff and patients in developing and implementing Quality Improvement Plans.Work interdependently with medical staff and other professionals while maintaining respect for professional boundaries.Maintain accurate and complete patient records.Encourage family participation in the prevention of falls and encourage good bone health.Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence.Establish links with other organisations that provide a similar nurse led clinic.Foster good relationships and liaise with patients and their families with GP and Public Health Nurses.Participate in case review in relation to bone health with relevant colleagues both primary and secondary care as appropriate.Take a proactive role in the formulation and provision of evidence based PPPGs relating to Bone Health ensuring that all policies, procedures, protocols and guidelines are research/evidence based and that they are updated as required.Take a lead role in ensuring Bone Health is in line with best practice guidelines and liaise with outside bodies associated with national guidance and best practice.Maintain patient records in accordance with local service and professional standards.QualificationsSelection Criteria:Selection criteria outline the qualifications, skills, knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.
