A panel may be formed as a result of this campaign for Social Care Leader from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.
The purpose of the recently-established role of Social Care Leader within the Primary Care Network Team in North Limerick City is to ensure that the experiences, preferences and needs of patients and service across all socio-demographics within the CHN are heard and responded to.
The establishment of the role is in direct response to a need identified by the community and a wide range of service providers identified through a comprehensive engagement project led by the CHNM and Patient & Service User Engagement Lead.
The post holder will work in partnership with the Primary Care Team across the CHN to continue to take an inclusive population based approach to delivering Primary Care Services with an emphasis on marginalised communities within North Limerick City.
The Office will maintain close links with other functions providing support to the population of the Network such as the Social Inclusion, Mental Health, Disabilities, the newly established Office of Patient Partnership and the wider HSE services.
This is to ensure that CHN 8 North Limerick City is represented on the future establishment of a proposed future patient council at Regional level in line with the Health Regions Programme Team's Proposal for Patient and Service User Partnership with Health Regions across all divisions.
A core function of the Social Care Leader role is to understand the barriers identified through comprehensive engagement completed through the North Side Engagement Project and work in partnership with the Network Manager, Primary Care Team, External Service Providers, Patient & Service User and the wider community to implement the key recommendations co-produced within the Network to address the barriers identified.
