The Flushing YMCA is seeking a Case Manager.
The Case Manager will provide participants with guidance and support and to advocate on their behalf and ensure they gain access to services that can help them succeed in school.
The Case Manager will work with the Community Schools population at JHS 189 and teen population at the Flushing YMCA.
The Case Manager will work one-on-one with middle school & high school students, as well as lead and run SEL groups.
This qualified individual will be managing the social, emotional and vocational aspects of participants in the program.
The Case Manager will provide on-going case management and support while coordinating services to determine academic, work readiness, occupational skills, and interests.
The Case Manager will provide students with a positive mentor relationship of advocacy and support to help them achieve their goals.
Key Responsibilities:
Ensure the health, safety, and wellbeing of participants in the program by providing close supervision of all activities.
Meet regularly with youth in groups and individually.
Guide clients in the development of skills and strategies for dealing with their problems or overcoming challenges/obstacles.
Manage case load of 20-40 youth participants.
Monitor student's academic, social and emotional progress.
Maintain contact with members of case load.
Schedule one-on-one appointments with participants.
Keep accurate attendance and youth services records.
Provide case management and refer to outside resources and agencies as needed.
Conduct initial case management meetings and assessments of participants and develop plans to ensure success.
Facilitate SEL groups and workshops based on the expressed needs and interests of participants to improved attitudes, school performance and home and community relationships.
Motivate participants and coordinate academic and occupational learning services and college and career exploration.
Actively participate in staff meetings and trainings and branch events.
Act as client's advocate to coordinate required services or to resolve emergency problems in crisis situations, refer patients, clients, or family members to community resources or to specialists as necessary; and coordinate referrals to indicated services inside and outside the youth and family department (e.g., social services, mental health specialty care, substance abuse treatment).
Guide participants in the development of skills and strategies for dealing with their problems or overcoming challenges/obstacles.
Lead and/or organize trainings, workshops and other learning experiences that help prepare participants for workforce and educational experiences, as part of their individual work plan.
Assist with organizing and coordinating after school activities & program newsletter.
Report weekly achievements to direct supervisor.
Network with organizations in the community.
Maintain and develop positive relationships with families, participants, staff, schools, volunteers, other organizations.
Attend outside meetings, conferences, workshops, and other networking opportunities to promote organization visibility.
Other duties/task may be assigned as needed.
Desired Skills & Experience:
BSW, MSW or working towards MSW is required.
Minimum of one (1) year of experience working in youth programs.
Experience teaching and facilitating training sessions on issues related to youth and teens.
Must be CPR/First Aid certified and complete the YMCA Mandatory Reporting Training within four (4) months of employment.
