About the Role:
The purpose of the job is to promote and support independent living, community integration, personal development, social interactions, and work opportunities for individuals who are part of RehabCare services. The post will focus on assisting in the delivery of individualised community support packages to individuals with a history of mental health challenges, including those with Autism or physical and sensory disabilities.

Your Responsibilities:
- Plan, develop, implement, monitor, and review appropriate educational, recreational, and occupational programs within the community.
- Ensure best practice in line with up-to-date research regarding Quality-of-Life improvements.
- Liaise with families, the multidisciplinary team, and key community-based personnel in the pursuit of accessible community activities for service users.
- Ensure all interaction with service users is approached with dignity, respect, equality, and incorporates choice.
Additional Requirements:
- QQI level 5 in Health & Social Care or equivalent.
- Previous experience of working in the Community.
- Experience of working 1:1 in the community.
- Understanding and knowledge of intellectual, physical and/or mental health disability.
- Full clean driving licence and access to a car for business purposes is required.
If applicable, candidates will need to provide a police certificate for any country they've lived in before moving to Ireland.
A panel may be formed as a result of this campaign.
