OverviewSocial Worker, Palliative Care, Oc.KildareCpl Healthcare, in partnership with the HSE are recruiting for an experienced Social Worker to work in a specialist palliative care service based in Co.Kildare.You will work as an integral part of a multidisciplinary team, based in a 16 bedded inpatient unit which provides symptom management, pain control, respite & End of life Care and will provide psychosocial support, guidance & follow up contact.You will also provide a Social Work Service to clients in the community linked in with the Palliative Home Care Team in a sensitive and holistic way aimed at improving the quality of life for people living with life limiting conditions and to provide comfort and dignity at end of life.ResponsibilitiesProvide a medical social work service for clients of the specialist palliative care service, including assessment, crisis intervention, counselling, family work, discharge planning with other team members, and helping patients and families access health, social and welfare entitlements in the community.Work closely with other team members in responding to the social and emotional impact of terminal illness and hospitalisation on patients and their families.Bereavement follow up where appropriate.Areas of ExperienceCounsellingPatient & family supportPsychological assessmentDischarge planningLinking in with external agenciesKnowledge of working with the Nursing Home Support / Fair Deal SchemeThe CandidateA recognised qualification as a Social Worker with CORU registrationPrevious experience in a medical setting – placement experience will be consideredPermission to work in IrelandProfessional approach to workFlexibility in work practiceHow to ApplyFor further information, please forward your CV to ****** FOA : Kami#J-*****-Ljbffr
