Responsibilities:

  • Conduct comprehensive psychosocial assessments and develop individualized care plans for older adults.
  • Provide therapeutic interventions and support to patients and their families.
  • Collaborate with multidisciplinary teams to deliver holistic care.
  • Advocate for patients' rights and access to services.
  • Maintain accurate and up-to-date case records and documentation.
  • Participate in team meetings, case conferences, and training sessions.

Requirements:

  • Minimum of a level 8 degree in Social Care.
  • 3 years post qualifying experience to work at Senior Social Worker level 8.
  • CORU registration.
  • Experience in mental health services, preferably with older adults.
  • Strong assessment, intervention, and case management skills.
  • Full driving license and access to own car is desirable.
  • Excellent communication and interpersonal skills.
  • Resident in Ireland and legal right to work in Ireland without restriction.
  • All candidates will be required to undergo Garda Vetting.
  • International Police Clearance (if lived outside of Ireland for more than 6 months after the age of 16).
