Job Description

Summary

We are hiring a Physical Therapist to join our team in Senior Living at Brookdale Trillium Crossing in Dublin, OH. This is a full-time position with bonus opportunities.

Responsibilities:

  • Evaluate patients to determine rehab potential, identify needs, develop individualized treatment plans, and establish goals for team care
  • Set initial short-term and long-term goals
  • Use equipment safely, maintain logs for equipment inspection, maintenance, and cleaning, and report any broken or faulty equipment
  • Maintain logs for required outpatient clinic equipment
  • Follow medical regimens and adhere to precautions (e.g., THR, TKR, cardiac, wound care)
  • Perform safe treatment techniques such as transfers, positioning, feeding, splinting, paraffin baths, cryotherapy, ultrasound, and electrical stimulation
  • Complete discharge summaries on the final visit for each patient

Education and Experience:

  • Current Physical Therapist licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, access to a reliable vehicle or public transportation
