Clinical Nurse Specialist (Psychiatry of Later Life) role in the National Mental Health clinical programme. The purpose of the post is to deliver effective, recovery-oriented, high-quality, team-based, integrated care for older persons with mental health problems. The role includes assessment and support to individuals through appropriate treatment and recovery-oriented interventions to support care in the community and reduce the need for acute hospitalisation. This will include domiciliary visits, family support and liaison with other services.
The post involves working as a member of a community mental health team, undertaking standardised assessments and management, with assertive follow-up involving families and carers as appropriate. The post operates within the defined scope of practice and aligns with the five core concepts of the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th Edition, NCNM (2008).
- Deliver care in line with the five core concepts of the CNS role as outlined in the NCNM framework.
- Undertake assessments and initiate a range of evidenced-based recovery-oriented clinical interventions.
- Link with statutory services in secondary and primary care, and with voluntary services in the local community.
- Work as part of a community mental health team with standardised assessments and management, including follow-up with families/carers as appropriate.
- Provide domiciliary visits and family support, and liaise with other services to coordinate care.
The caseload comprises service users attended by the identified community mental health team or service and will be within the post's scope of practice.
