Job Title

Client Care Co-ordinator South Dublin City / Kildare

Overview

We are looking for a highly motivated and experienced Client Care Co-ordinator to join our team. You will be responsible for managing and coordinating care services for clients in South Dublin City and Kildare. You will work closely with clients, their families, and our team of care professionals to ensure that the highest level of care is provided.

Responsibilities
  • Coordinating and managing care services for clients
  • Liaising with clients, their families, and care professionals to ensure the highest level of care is provided
  • Developing care plans and ensuring they are implemented effectively
  • Scheduling and managing staff rotas to ensure adequate coverage
  • Ensuring all care records are up-to-date and accurate
  • Responding to client queries and concerns in a timely and professional manner
  • Conducting regular client reviews and updating care plans as necessary
  • Providing support and guidance to care professionals
  • Ensuring all care services are delivered in accordance with company policies and procedures
  • Participating in the on-call rota as required
  • The role will involve provision of care to clients and requires the post holder to be fit to work in this capacity and hold the relevant care qualifications for QQI Level 5 in Care Skills and Care of the Older Person
Requirements
  • Proven experience in a similar role in the healthcare industry
  • Excellent communication and interpersonal skills
  • Strong organisational and time-management skills
  • Ability to work well under pressure and in a fast-paced environment
  • Ability to work independently and as part of a team
  • Good IT skills, including proficiency in Microsoft Office
  • Knowledge of healthcare regulations and legislation
  • Full driving licence and access to a car (travel allowance, parking and tolls full paid)
  • Must be eligible to work in Ireland without restriction
  • QQI Level 6 in Supervisory management or equivalent recognised qualification
  • Familiar with operation of OneTouch Health platform

Location & Hours

Location: Office in Kimmage, Dublin 12. The role involves providing care to clients in Dublin South City and Kildare.

Hours: Monday to Friday, 9:00 am – 5:00 pm. An on-call rota is in place with additional allowance for weekend on-call work (remote from home).
