Headway is an internationally accredited service organisation that supports adults who are affected by an Acquired Brain Injury, whether from stroke, traumatic head injury or other cause. With over 80 staff throughout the country, Headway serves over 1200 clients and family members and answers around 1000 helpline queries per annum. Headway provides services to clients from five locations in Cork/Kerry, four in Dublin city, and four in the Limerick region, as well as through the service of community rehabilitation case workers, counsellors, and outreach. We are currently seeking a Community Rehabilitation Case Worker to join our team in Cork. This is a one-year contract role, covering a period of maternity leave, and the role will be based in our Ballincollig and/or Carrigrohane centre and in the community. This is a full-time role, 35 hours per week MondayFriday (time in lieu for evening/weekend appointments if required). Point on salary scale depending on level of experience. Role Summary: Facilitating clients to develop an individual goal plan and to participate in activities within their own community. Preparation and facilitation of group activities and programmes. Providing information and support to clients and carers. Helping clients to overcome difficulties or barriers they encounter in their day-to-day activities, increasing confidence and independence through practical involvement. Working as a member of the overall Cork Rehabilitation Team, participating in providing support and services to existing clients, including facilitating in-service groups and providing cover on the Rehabilitation staffing calendar if required. Support of clients in the community in dealing with every day and new activities without barriers, and in accessing supports in the community. Presentations to a range of organisations and networking in the community. Main Responsibilities: Assess the needs of clients in relation to community reintegration. Identify the clients interests, capacities and needs. Identify the type of activities preferred by and suited to the client. Working closely with the client, and carer(s)/other stakeholders if applicable, assess the clients aspirations, experiences, abilities, skills, knowledge and any potential obstacles. Work with clients exiting Rehabilitation Training programmes to develop and support a weekly timetable of community activities. Develop and facilitate group programmes and activities. Work with caseload of direct Community Rehabilitation referrals to develop a specific strategy leading to community activities, in line with individual client need. Develop a network of disability friendly contacts for activities and placements as needed. Ensure that the client is an active participant in all phases of the process. Facilitate clients to take a lead in their own lifestyle choices and to implement these to the best of their ability. Identify potential suitable organisations by matching the needs of the organisation to the needs, abilities and skills of the client. Carry out access audits for client caseload as required. Provide support tailored to the client in activities in the community. Assist with travel arrangements, including travel training, for the client as required. Support clients accessing voluntary/work placements as required. Monitor the clients progress and endeavour to resolve any issues that may arise. Provide any support required in integration (e.g. mentor/buddy system, disability awareness training, personal care supports). Preparation and presentation of group activities and programmes appropriate to the client group. Take responsibility for the needs of clients in line with relevant health and safety legislation and Headway policy/procedure. To include: Take responsibility for the physical care of clients as required (including assistance with toileting needs). Administer first aid if qualified to do so (at a level appropriate to post holders training and ability). Maintain a safe and pleasant environment at all times. Skills and Experience: Degree level qualification in social care/rehabilitation or other relevant discipline and experience working with people with disabilities. Experience working in the community an advantage. Full clean drivers licence and access own car for work is an essential requirement for this role. Strong group facilitation and client support experience. Excellent communication and interpersonal skills. Reliability and a flexible approach. Excellent administrative, organisational and PC skills. Strong client centred, ethical approach. The ability to motivate and assist/support individuals with ABI. The ability to develop networks in the community and to act as advocate for Headway and its clients. The ability to work both on own initiative and as part of a multi-disciplinary team. Creativity (e.g., in using community resources, problem solving, dealing with client needs). Professional, accountable, good planning and time management skills.
