Healthcare & Wellbeing Coordinator Organisation: Society of the Sacred Heart, Central Europe & the Islands Province Area: Dublin, Ireland Salary: Commensurate with qualifications and experience Hours: 3-days per week (7 hours per day). Place of work: Home based Reports to: Provincial About the Society of the Sacred Heart The Society of the Sacred Heart is an international community of women in the Catholic Church, founded in France in 1800 by Saint Madeleine Sophie Barat, and established in Ireland in 1842.? The Society in Ireland is part of the Province of Central Europe & the Islands (CEI), that comprises Ireland, Scotland, England, Wales, Austria, Hungary, Germany & Malta. The successful applicant will be working in Dublin, Ireland. The Society first established a Convent and Schools at Mount Anville in 1865, and for over 150 years, educated more than 10,000 pupils at Mount Anville Schools. In 2007, the Society passed the trusteeship of the schools on the Mount Anville campus to the Mount Anville Sacred Heart Education Trust (MASHET). Today, there are 16 Sisters living in the Rep of Ireland with an average age of 83 years, all retired. About the role The successful candidate will seek to ensure that the Sisters living in Dublin have a good quality of health and well-being, by providing them with the necessary supports and information. The post holder will be an empathetic and motivated individual who is able to work flexibly and organise his/her time based on the individual needs of the individual Sisters. They will also need to be a confident driver with a full driving licence and the use of a private motor vehicle, with current tax and NCT. The role does not extend to personal care which will continue to be provided by other agencies, e.g. HSE/Dovida. Competencies: Always Observe a Duty of Care Adept at building relationships and fostering mutual respect and trust Work within best practice and observing privacy and confidentiality Establish systems and policies relating to healthcare that Sisters can have confidence in Good time management with a strong emphasis on effective organisation and planning Strong interpersonal skills to include compassion, sensitivity, and recognition of non-verbal indicators within a conversation. Responsibilities: To provide holistic assessment and ongoing review of the health and well-being of sisters, supporting them to continue living independently in the community. Duties may include: Contacting sisters and by agreement visiting them at home to provide confidential advice and support with any matters of health and wellbeing they wish to discuss, whilst being sensitive to the different levels of input required by individual sisters Ensuring that sisters have access to local health services and support structures, identifying and arranging support from specialist health or social care teams where available - and following up on referrals and appointments on behalf of sisters, if required Accompanying sisters to medical or other health related appointments, including GP & hospital appointments, tests, diagnostics investigations and clinical procedures, when/as required Supporting sisters at appointments by seeking clarity and asking relevant questions from clinicians to achieve the best outcomes Ensuring the sister understands the outcome of the appointment, interpreting medical jargon and highlighting main points in a written summary, if required Reporting relevant changes, developing needs or concerns about a sisters health and wellbeing to the Provincial, and if appropriate, the Safeguarding Lead, relevant staff or Provincial Team member Being aware of individual sisters needs or health conditions and the impact they may be having on other members living in a community Working with the sister, members of her community and/or the Provincial when advice and support is required to cope with conditions such as dementia Liaising with hospital, social services, community health staff on behalf of a sister (or the Provincial) for those who are admitted to or being discharged from hospital Keeping in touch with those on regular medication, liaising with GPs and pharmacists as required to ensure medications are dispensed and taken as prescribed, and regularly reviewed Liaising with and updating the Provincial (or relevant staff member) if necessary, about a sister, having first obtained her consent and sharing only the necessary information, whilst respecting the need for confidentiality. Keeping excellent written records of all work. Meeting with the Senior Health Advisor as required for structured support and supervision. Keeping up to date with CPD as required. The post holder may be required to perform other duties as appropriate to the post, which may be assigned to him/her from time to time by the Provincial or another designated officer. Essential criteria: Live Registration, annually renewed with An Board Altranais The clinical and administrative capacity to properly discharge the functions of the role Relevant qualification in Geriatric Medicine or at least 2 years experience in the discipline A minimum of two years management experience in an older persons healthcare setting Compassionate, empathetic, with a strong level of emotional intelligence Ability to be flexible in relation to hours of work, and to attend some meetings out of normal work hours. Proficient in Microsoft outlook email and word for report writing Garda Vetting: The successful candidate will be required to satisfactorily complete Garda Vetting. Terms of the Contract This is a 2-year part-time fixed-term contract of employment with 22 days (pro-rata) paid annual leave. Access to the Societys PRSA Scheme with an employers contribution of 5% basic annual salary. Travel expenses will be paid at a rate equivalent with RoI civil service rates, and vouched expenses paid in accordance with the Societys expense reimbursement policy. Salary commensurate with successful candidates qualifications and experience.
