Physical Health Practitioner / Nursing Associate

To be an integral part of the Physical Health Care Team within the Lincolnshire Early Intervention in Psychosis (EIP) service. Supporting patients aged 14-65 years old in community settings to access high quality physical healthcare assessment, care plans and interventions.

Offering physical healthcare monitoring under the direction of the EIP Physical Healthcare Lead within the community settings around the East of the County however being countywide may also been required at times.

The post holder will support the wider nursing team to provide holistic and person centred physical healthcare monitoring and low level psychologically based interventions to make necessary improvements.

Main duties of the job

Provide physical healthcare monitoring interventions with service users in the countywide including providing advice to patients

Monitor the condition and physical health needs of people within their care on a continual basis in partnership with service users, families and carers

Contribute to ongoing assessment, recognising when it is necessary to refer to others for reassessment

Carry out care packages related to the physical health aspects of planned care

Recognise the contribution of others in the provision of service user care

Involve service users/ carers in the planning and delivery of care in a compassionate and appropriate manner which considers their diverse needs

Maintain the welfare and safety of service users and protect them from any form of abuse in accordance with safeguarding guidance

Demonstrate a knowledge of information governance and the need to protect service user confidential information

Make and maintain accurate records of monitoring interventions in accordance with Trust policy

Participate in regular clinical supervision

Engage in public health, community development and in the reduction of health inequalities

Be able to identify role boundaries

Leadership

Work in partnership with multi-professional colleagues or agencies

Contribute to innovation and new ways of working

Take part in local audit and research activities where appropriate

Maintain a professional role

Job responsibilities

Responsibilities for direct/indirect patient care

- Provide physical healthcare monitoring and health promotion interventions, implementing care packages under the direction of the Registered Practitioner
- Report and escalate concerns or abnormal outcomes of monitoring interventions to the Registered Practitioner or Responsible Clinician
- Monitor and contribute to the review of service user progress where frequent concentration is required against planned objectives and use initiative to act on the results within the sphere of ones own responsibility, including reporting observations and taking action to minimise risks
- Implement planned care which encourages and empowers service users and carers to maximise self care and autonomy under the direction of the Registered Practitioner
- Contribute to case conferences and reviews, supporting service users and carers to engage and contribute to their care
- Engage in the public health, community development and in the reduction of health inequalities

Responsibilities for policy and service development implementation

- Adhere to legislation, policies, procedures and guidelines both local and national. Regularly attend workplace and staff engagement meetings and contribute positively to discussions about the improvement of care

Responsibilities for financial and physical resources

- Exercise personal duty of care in the safe use and storage of equipment

Be environmentally aware and prudent in use of resources and energy

Person Specification

Qualifications

- GCSE Grade A-C or Grade 9-4 in Maths and English or functional skills level 2 Maths and English or equivalent
- Diploma or equivalent qualification in a health related subject, eg. foundation degree, or
- NVQ Level 3 plus short courses and relevant experience to Diploma level
- Registered Nursing Associate on the NMC register
- Care Certificate

Experience

- Previous experience of working within a health / care setting
- Understanding the context of multidisciplinary team working
- Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
- Understanding of the importance of following procedures and treatment plans
- Experience of working in a physical healthcare environment/role
- Experience of working with patients with Severe Mental Illness

Skills

- Ability to support the development, implementation and evaluation of physical healthcare interventions of patients/service users
- Be skilled in carrying out basic physical healthcare interventions including: physical observations, , removal of wound, blood glucose monitoring, venepuncture, ECG recording
- Consider each service user's personal circumstances and preferences in order to implement and adjust programmes of care and intervention, in conjunction with the wider multi-disciplinary team
- Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
- Evidence of time management skills and ability to prioritise delegated workload
- Ability to communicate effectively with patients/ relatives and carers and all memberso fthe multi-disciplinary team
- Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact
- Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
- Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
- oExperience of use of Point of Care Teasting Equipment

- Be able to travel independently between sites without the use of public transport
