Hours of work: Part time, 30 Hours per week (0.8 WTE)

Position Summary: In partnership with members of the multidisciplinary nursing team, the post holder will develop, implement and evaluate a seamless specialist urology service, ensuring that patients receive the highest standard of clinical care. They act as a lead specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines, and contribute to service developments in collaboration with multidisciplinary colleagues. Also ensures the safety and well-being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person-centered care principles.

Responsibilities

Responsibilities:

1. Collaborate with urologists and other healthcare professionals to assess patient's suitability for bladder treatments and monitor the progress of therapy.
2. Provide care and education for patients with chronic bladder conditions such as interstitial cystitis.
3. Provide information on bladder health, continence care, and catheterisation and lifestyle changes to improve their quality of life.
4. Prepare and assist patients for bladder instillation procedures, ensuring proper catheterisation and sterile techniques to minimise infection risk as per SARI Guidelines.
5. Administer bladder instillations for patients undergoing intravesical therapy, including Cystistat, Gentamycin and DMSO.
6. Monitor patients for any adverse reactions or complications post procedure, including bladder irritation, infection, or other side effects including pain management and patient comfort strategies.
7. Work autonomously; document all patient assessments, treatments and progress.

Clinical Focus – Direct Care:

1. Provide direct, specialist nursing care to patients with various urological conditions, including bladder infections, bladder pain and incontinence who require support and treatment through the continuum of care.
2. Undertake comprehensive patient assessment to include physical, psychological, social and spiritual elements of care using best evidence-based practice in urology.
3. Use the outcomes of patient assessment to develop and implement plans of care/case management in conjunction with the Urology consultants, multi-disciplinary team (MDT) and the patient, family and/or carer as appropriate.
4. Monitor and evaluate the patient's response to treatment and amend the plan of care accordingly in conjunction with the MDT and patient, family and/or carer as appropriate.
5. Accept appropriate referrals from MDT colleagues.
6. Co-ordinate investigations, treatment therapies and patient follow-up.
7. Communicate with patients, family and/or carer as appropriate, to assess patient's needs and provide relevant support, information, education, advice and counselling as required.
8. Participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management.
9. Identify and promote specific symptom management strategies as well as the identification of triggers which may cause exacerbation of urological symptoms. Provide patients with appropriate self-management strategies and escalation pathways.
10. Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence. This will include the provision of educational and health promotion material which is comprehensive, easy to understand and meets patient's needs.
11. Ensure that patients receive high quality clinical care and a good patient experience.

Indirect Care:

1. Identify and agree appropriate referral pathways for new and existing urology patients.
2. Participate in case review with MDT colleagues as appropriate.
3. Use a case management approach to patients with complex needs in collaboration with MDT in both Primary and Secondary Care as appropriate.
4. Assist with the set-up of appropriate clinics and procedures for assessment and follow up of urology patients.

Education and Training:

1. Act as a facilitator/mentor/preceptor to student nurses, nurses and others teaching new skills to aid the expanded role of the nurse.
2. Assess own educational needs and take steps to keep up to date with clinical and nursing developments in line with requirements of NMBI and Beaumont Hospital.
3. Involved in pre-registration and post-registration nursing and medical programmes.
4. Ensure that the learning environment and specialist training is appropriate for both pre and post registration learners and participate in the development of appropriate programmes.
5. Co-ordinate the development and delivery of a patient-focused education and information giving strategy within the specialty.
6. Motivate, develop, support and identify training needs for department staff in conjunction with CNMs/CPSN.

Audit and Research:

1. Promote and undertake nursing research and publish the outcome, updating own knowledge to promote excellence in clinical practice.
2. Utilise research findings in the delivery of specialist patient care, developing new ways of working and disseminating relevant information to staff.
3. Participate in and be aware of other clinical research trials for patients.
4. Lead and contribute to audit and research within the specialty in collaboration with colleagues, developing action plans and disseminating information.
5. Familiar with current clinical research protocol issues and relevant treatment and procedures employed in the diagnosis and treatment within their specialty.

(a) Eligible applicants will be those who on the closing date for the competition:

1. Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.
2. Have at least 5 years post-registration experience (or an aggregate of 5 years full-time post-registration experience) of which 2 years must be in the specialty or related area.
3. Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
4. Demonstrate evidence of continuous professional development.

(b) Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.

1. Annual registration

(i) On appointment, practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).

(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

- A management course.
- A urology/ oncology course or willing to undertake.
