Person Support Coordinator

Overview The Person Support Coordinator with initial designation to Individualised Skills Development and initially located in Wexford, will support individuals to develop specified skills to enable and empower them to live a more fulfilled and independent life and exercise their rights and lifestyle choices. In this regard they will have a high capacity to build relationships, have excellent interpersonal and communication skills. They will be working 1-1 with each individual which requires them to have the ability to work on their own initiative within the remit of the role to have timely follow-up to achieve completion of identified goals. They will work and respond flexibly to support individuals in line with their will, preference, choice, and rights. Purpose The Person Support Coordinator will support individuals to develop the skills, confidence and potential to live a more independent life. Due to the nature of the support role in the community and the lone working element, regular interaction with their Line Manager will support in their development in the role, while they focus on certain aspects related to the further development of each individual. The Role Outline The Person Support Coordinator will be passionate about creating opportunities for people with disabilities to develop their skills and abilities to live a life of their choosing and independently engage and be part of their community. Therefore the postholder will have experience developing trustworthy individual relationships and enabling each person to develop their confidence and be life skills. They will have experience building connections with community and voluntary groups, statutory agencies and local employers for to support individuals achieve the universal good things of life. Qualifications & Experience The ideal candidate shall have: A third level qualification in social care or related fields Full valid drivers license and access to a vehicle. High level of proficiency in English, both written and verbal. Experience supporting skills development with adult learners with intellectual or adaptive disabilities . Experience in supporting individuals to have the life of their choosing in the community. Familiarity with adult learning theories and principles Strong communication and interpersonal skills The ability/understanding to assess individuals learning support needs and learning styles, and to adjust supports accordingly Proficient in delivering on individual outcomes related to their learning/skills building plans Excellent organisational skills Excellent IT skills and proficiency in using technology to enhance learning The Role The Person Support Coordinator requires excellent interpersonal, communication and reflectibve practice skills, with the capacity and track record of working on own initiative and deliver on measurable, achievable goals. They will have an entrepreneurial ability, together with drive, enthusiasm and a positive attitude and will be expectedto act in an open and fair manner, using a consistent approach in supporting each individual. Working through St. Margarets Framework of Supported Self-Directed Living, they will: Engage and develop a relationships each individual to support them to achieve their goals, underpinned by St. Margarets culture and core values of Partnership, Rights, Inclusion and Accountability. Work within the agreed learning framework to develop learning plans for each individual Assess skills and learning needs to create learning action plans and goals with each individual. Develop tailored plans with each individual which will outline their specific outcomes (goal achievement) that are measurable and achievable. Create a supportive and inclusive environment that respects diverse backgrounds and learning styles Use and resources to engage each individual and facilitate their learning Provide supports to enable the individual to achieve their goals Collaborate with other stakeholders and the circle of support to improve outcomes for the individual. Monitor and evaluate progress through ongoing assessment with the individual and provide timely reports to the Line Manager. Maintain accurate records of individuals engagement, progress and achievements. Regularly meet with and update the Line Manager on progress, opportunities, challenges and blockages Report issues of concern to the Line Manager (e.g. Safeguarding, Rights Restrictions, etc.) Stay updated on the latest best practices relating to skills building for people with intellectual and adaptive learning needs. Role Requirements The postholder will be able to work at a high level of autonomy. Positive mind-set and passion to support others to live a self-directed life. Good time management. Clearly understand their role boundaries in the programme Deliver on agreed goals and individual outcomes Complete documentation and reports in a timely manner Excellent communication, both written and verbal, with the participants, families, other stakeholders and the Line Manager. The postholder may be required to be flexible in terms of working hours, depending on the service and support needs of the individuals in the programme. NOTE: Weekend and sleepovers are a requirement as a Person Support Coordinator, however they are not a requirement while undertaking the designated role in Individualised Skills Development. However, such flexibility may be required in service in emergency circumstances. PARTICULARS OF OFFICE The appointment is initially on a fixed-term 2 years. The contract will 39 hours per week. Role, Responsibilities & Accountability The Person Support Coordinator will perform the role and responsibilities of the post outlined in the Job Description and assume any other responsibilities that are applicable to the full discharge of the post. The Person Support Coordinator shall be accountable for their professional practice, working with each person they support and as a member of the Person Support Team. Hours of work: Normal working hours are 39 hours per week on a 5 rota Monday to Friday.
