Responsibilities Maintain patient/resident confidentiality at all times.
Assist residents with all activities of daily living.
Conduct assessments, plan, implement, and evaluate social work programs for residents according to professional standards.
Manage patient care to ensure the highest professional standards, utilizing a personalized approach.
Communicate and coordinate with residents, staff, and family members to provide the best possible care.
Participate actively in multidisciplinary healthcare teams and work collaboratively as part of the team.
Keep accurate records of assessments, interventions, and communications related to residents.
Qualifications A relevant qualification at Level 7 or higher in Social Care or hold a CORU registration under the Social Care Worker division.
Bachelor of Arts in Social Studies.
Bachelor of Arts in Applied Social Studies in Social Care.
Bachelor of Arts in Applied Social Care.
Bachelor of Arts (Honours) in Social Care.
