Hours of work: 35 Hours (full time)

Position Summary: The Clinical Specialist Podiatrist will have responsibility for the provision of a high quality, evidence-based podiatry service to patients presenting with active Diabetic foot disease. They will oversee the development of the Diabetes podiatry service and provide a limited non-diabetes service to patients with active foot disease during consultant lead clinics only, until specialist-specific podiatry posts are in place. The clinical specialist podiatrist will work closely with the Diabetes multi-disciplinary foot team (MDFT) to develop and deliver inpatient and outpatient services.

Responsibilities

Key Responsibilities:

- To maintain and develop high standards of practice and specialist knowledge in the clinical area of diabetes podiatry across the continuum of care.
- Provide standardised high quality diabetes podiatry management to patients with active foot disease, Charcot arthropathy in line with the care plan developed by the MDFT.
- Develop individualised management plans following an in-depth accurate assessment and diagnosis of the patient, using highly advanced and specialist skills in podiatric management.
- To develop and implement discharge and after care policies and ensure good liaison with home care, primary, community and continuing care.
- Ensure timely referral to other services in the foot protection team or others as required.
- Adhere to national and international guidelines on the management of the diabetic foot.
- Develop relationships and collaborate with new Community podiatry hubs, coordinating patient care, developing treatment plans. Mentoring new community staff developing links between Level 4 hospital and community podiatry.
- Represent the podiatry department providing leadership helping to develop and improve the hospital podiatry service. Attending meetings as required to facilitate service development.
- Communicate / Liaise as appropriate with the MDFT and community chronic disease specialist teams which include but not limited to; general practitioners, practice nurses, clinical nurse specialists, clinical specialists (CS), senior and staff grade podiatrists; CS, senior, staff grade Dietitians and community Pharmacists. Multidisciplinary team including but not limited to: Consultant Endocrinologists, Vascular, Orthopaedics, Renal, Infectious Diseases (ID), Advanced Nurse Practitioner (ANP), Diabetes Nurse Specialist (DNS), Tissue Viability Nurse (TVN), OPAT nurses, Orthotist, Social work, Physiotherapists, Occupational therapists, and Podiatrists.
- Participate in education and training sessions of community podiatrists and practice nurses and other health care professionals involved in the care of the diabetic foot.
- Participate in the training of student Podiatrists.
- To manage clinical risk within own caseload and that of designated teams.
- To ensure appropriate pathways of care are defined.
- To demonstrate sound understanding of clinical governance and to demonstrate the use of treatment plans with evidence-based links.
- To record client related information in accordance to Beaumont Hospital departmental and hospital guidelines and professional standards.
- Provide data reports of hospital-based statistics to the clinical and Medical Directorate lead and the National Diabetes Programme as required.
- Ensure professional standards are maintained in accordance with CORU.
- Optimise the use of available resources to achieve effective outcomes when planning and delivering a flexible service that meets the needs of the podiatry patients under the care of Beaumont Hospital.
- To keep abreast of new developments (e.g. clinical care programmes, health policy, best practice guidelines), skills and technology to ensure continued competency in the post.
- To ensure that high quality patient centred services are based on the best available resources. To develop and implement service/business plans, quality initiatives, audits etc. and reporting on outcomes in collaboration with Clinical Directorate Lead and Medical Directorate Lead.
- Produce an annual service plan/report.
- Partake in regular audit of the service.
- Develop and present health promotion packages for service users/stakeholders.
- To provide leadership and direction to diabetes team activities and to represent the podiatry profession confidently within service area and Beaumont hospital.

Qualifications

Eligibility + Selection criteria:

Mandatory:

1. Statutory Registration, Professional Qualifications, Experience, etc:

1. Eligible applicants will be those who on the closing date for the competition:
2. Be registered, or be eligible for registration, on the Podiatrists Register maintained by the Podiatrists Registration Board at CORU.
3. Have 5 years full time (or equivalent) post qualification clinical experience of which 4 years must be consecutive in the required area of specialism.
4. Must demonstrate evidence of continuing professional development relevant to the required area of specialism, in the form of post-graduate qualifications or courses relevant to a specialist diabetes foot service.
5. Candidates must demonstrate achievement in the areas of clinical audit, quality improvement initiatives, practice development, teaching and research.
6. Provide proof of Statutory Registration on the Podiatrists Register maintained by the Podiatrists Registration Board at CORU before a contract of employment can be issued.
7. Candidates must possess the requisite knowledge and ability, (including a high standard of suitability, management, leadership and professional ability), for the proper discharge of the duties of the office.

2. Annual Registration:

1. Practitioners must maintain annual registration on the Podiatrists Register maintained by the Podiatrists Registration Board at CORU.
2. Confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

3. Health: Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

4. Character: Candidates for and any person holding the office must be of good character.

Desirable:

- Continuing professional development (CPD) in Podiatry related to Diabetes.
- Relevant masters or Doctoral qualification.
- Member of relevant professional body.
- Significant clinical experience in diabetes podiatric care.
- Broad knowledge of other podiatric specialties.
- Research experience.
- Experience in podiatry coordinated care of patients with active foot disease.
- Experience in casting.
- Background knowledge of Beaumont hospital governance and organisational structure, core activities, values, and strategic aims.
