Patient Care Specialist

We are seeking a motivated and hardworking individual to join our multi-disciplinary team in providing rehabilitation and end of life care for people in the local community.
The successful candidate will have excellent communication skills, understanding of patient safety and ability to work as part of a team and on their own initiative.
Key Responsibilities:- Liaising closely with nursing and therapy staff to deliver person-centred care that positively impacts patients' health and wellbeing.
- Documenting and delivering any care provided with excellent communication and teamwork.
- Working flexibly, including days and nights on the ward.
- Excellent communication skills.
- Understanding of patient safety.
- Ability to work as part of a team and on their own initiative.
A 1-year fixed-term contract is available. Salary is quoted for full-time hours (37 hours per week). For appointments to part-time hours, salary will be pro-rata.
Additional Requirements:Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check. If the post is assessed as a 'regulated role', appointment will be subject to joining the Protecting Vulnerable Groups Scheme.
To work in the United Kingdom, there is a legal requirement for an individual to demonstrate they have the relevant permission to work in the country. NHS Scotland Boards will check entitlement to work in the UK as part of pre-employment checks.
