As a key member of the team, you will play a crucial role in the rehabilitation journey of clients with Acquired Brain Injury (ABI). The successful candidate will have a passion for supporting individuals and families affected by ABI.
The ideal candidate will have CORU registration as a Social Worker and experience working with ABI, disability, or hospital/community social work. They will possess strong communication, advocacy, and team collaboration skills, with a commitment to person-centred care and rehabilitation.
Main Responsibilities:- Discharge Planning: Collaborate with the multidisciplinary team to develop individualised discharge plans that meet the unique needs of each client.
- Housing Support: Assist clients and families with housing applications and related supports, ensuring a smooth transition to community-based living.
- Community Reintegration: Advocate for client needs within community and statutory services, promoting seamless integration and reconnection with their local community.
- Emotional Support: Provide emotional and practical support during transitions, empowering clients to maintain independence and confidence.
- CORU registration as a Social Worker.
- Experience working with ABI, disability, or hospital/community social work.
- Strong communication, advocacy, and team collaboration skills.
- Commitment to person-centred care and rehabilitation.
