Senior Specialist in Age-Friendly Care

You'll be a senior specialist member of the geriatrician team at this university hospital, playing a central role in identifying, admitting, overseeing, and discharging patients utilizing post-acute rehabilitation beds across the region.
This position supports development of integrated age-friendly care pathways in line with Sláintecare and HSE National Clinical Programme for Older People.
Clinical Responsibilities:- Lead multidisciplinary care of older patients across acute services including Frailty at the Front Door, Inpatient Geriatric Ward, Comprehensive Geriatric Assessment clinics, Orthogeriatrics, Outpatient services, Day hospital/ambulatory geriatric care.
- Provide inpatient consultations across medical and surgical wards.
- Participate in multidisciplinary team meetings and family case conferences.
- Collaborate with community services (public health nurses, GPs, reablement teams).
You will deliver expert care in Comprehensive Geriatric Assessment across acute inpatient services and rehabilitation settings, including off-site rehab beds. You will also lead multidisciplinary teams comprised of physiotherapists, occupational therapists, nursing staff, and community carers.
This is an initial short-term contract of 12 months, potentially extendable.
