As a Family Support Practitioner with TTM Healthcare Limited, you'll play a vital role in delivering high-quality care to individuals in need. Key Responsibilities:- Provide person-centred care to individuals based on their unique needs and preferences
- Assist with daily living tasks such as personal care and meal preparation
- Support individuals to engage in activities that promote independence and well-being
- Work collaboratively with multidisciplinary teams to achieve optimal outcomes
- Develop and maintain relationships with service users, families, and other stakeholders
- Minimum of Level 7 or 8 degree in Social Care
- At least 3 months social care experience
- CORU registration
- Full driving license and access to own car desirable
- Resident in Ireland and legal right to work without restriction
- All candidates will undergo Garda Vetting
- International Police Clearance if lived outside Ireland for more than 6 months after age 16
