Job Description:
We are seeking experienced and passionate professionals to join our team as Day Service Facilitators - Community Inclusion Specialists. As a key member of our service delivery team, you will play a vital role in supporting individuals with an intellectual disability to participate in a range of community-based activities that foster social inclusion, personal growth, and community engagement.
About the Role:
This is an exciting opportunity for a skilled and dedicated individual to make a meaningful difference in the lives of people we support. As a Day Service Facilitator - Community Inclusion Specialist, you will be responsible for planning and delivering person-centered programs that promote community integration, social participation, and individualized support. Your expertise in community development, capacity building, and advocacy will be essential in identifying opportunities for people to engage in meaningful activities and build their natural support networks.
Key Responsibilities:
- • Plan and deliver community-based programs that promote social inclusion, community engagement, and individualized support
• Collaborate with individuals, families, and community stakeholders to identify and pursue opportunities for community participation
• Develop and maintain relationships with community organizations, services, and resources to ensure seamless program delivery
• Provide individualized support and advocacy to people with intellectual disabilities, empowering them to take control of their lives and participate fully in their communities
• Work collaboratively with the service team to develop and implement person-centered plans that meet the unique needs and goals of each individual
Requirements:
To be successful in this role, you will possess:
- • A relevant Third Level Qualification in Social Care, Education, Community Development, or a related field (QQI Level 6 or higher)
• Minimum three years' experience of supporting adults with an intellectual disability or a similar field
• Proven expertise in community development, capacity building, and advocacy
• Excellent communication, teamwork, interpersonal, and organizational skills
• Ability to work independently and as part of a team, using own initiative and being flexible when needed
