Job Overview

This mental health professional plays a key role in a community-based team that delivers services for individuals struggling with serious mental illness.


Key Responsibilities:
  • Deliver comprehensive, community-based case management services and primary, behavioral health integrated services.
  • Respond to individual needs, desires, and interests using formal and informal assessment practices.
  • Develop and implement Person-Centered Plans for individuals consistent with guidelines established by relevant funding and accrediting entities.
  • Apply prevention, intervention, and resolution techniques tailored to the unique circumstances of each individual.

Qualifications:
  • Qualified Mental Health Professional (QMHP) certification.
  • Bachelor's Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or a related field.
  • Specialized training or one year of experience working with individuals with mental illnesses.
