About the Role:

We are seeking a highly skilled Care & Case Manager to join our team. The successful candidate will work closely with the Service Manager to ensure the day-to-day running of our service is smooth and efficient.

The Care & Case Manager will be responsible for processing referrals, conducting assessments, care/support planning and implementation of care/support plans. This role requires excellent organizational skills, attention to detail and the ability to work in a fast-paced environment.

Key Responsibilities:

  • Processing referrals and conducting assessments to determine the needs of our clients
  • Developing and implementing care/support plans that meet the individual needs of our clients
  • Working closely with the Service Manager to ensure the smooth operation of our service
  • Maintaining accurate records and reports

Requirements:

To be successful in this role, you will need:

  • A degree in social work or a related field
  • Experience working in a care management role
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
