About the Role

The position of Support Worker is a critical component in the delivery of high-quality support services at Praxis Care. Our team members play a vital role in empowering individuals with mental ill health, learning disabilities, autism, and dementia to live fulfilling lives.

Key Responsibilities
  • Develop and implement individualised support plans to promote independence and self-advocacy.
  • Work collaboratively with Team Leaders and managers to ensure seamless service delivery and effective communication.
  • Collaborate with colleagues to maintain a safe and respectful environment, addressing challenging behaviours with empathy and professionalism.
  • Administer medication as required, adhering to Praxis Care's Medication Policy.
  • Ensure accurate recording and logging of administrative tasks and relevant information.

Requirements
  • Fully participate in the company appraisal and staff review process.
  • Embody and model the company values, behaviours, and competencies at all times.
  • Perform duties according to Praxis expectations, regulations, and policies.
  • Be prepared to undertake additional responsibilities as needed.
  • Meet the necessary working hours to meet service needs.
