We are seeking an experienced Physiotherapist to join our multidisciplinary team in delivering high-quality rehabilitation services.

The Senior Physiotherapist will play a pivotal role in providing timely and effective interventions as part of our Early Supported Discharge (ESD) program.

This challenging position requires the post holder to work closely with healthcare professionals, supporting older adults transitioning from acute hospital care to home-based settings. The successful candidate will facilitate functional recovery, reablement, and optimal independence for patients.

Key Responsibilities:
  • Deliver comprehensive physiotherapy assessments and evidence-based interventions to older adults in community and home-based settings.
  • Contribute to safe discharge planning in collaboration with acute hospitals and community services.
  • Work collaboratively with multidisciplinary teams, including Occupational Therapists, Nurses, Medical Staff, Social Workers, and Case Managers.
  • Identify risks and implement strategies to minimize re-admissions and falls.
  • Monitor patient outcomes using standardized assessment tools.
  • Lead on care planning and documentation, ensuring high standards of clinical governance.
  • Provide supervision and mentoring to junior Physiotherapists and students.
Requirements:
  • CORU registration as a Physiotherapist.
  • Minimum 3 years post-graduate clinical experience, preferably in an older persons or rehabilitation setting.
  • Full clean driving license and access to a vehicle.
Desirable Qualifications:
  • Experience in ICPOP, ESD models, or integrated care settings.
  • Postgraduate training in areas such as falls prevention, neurological rehabilitation, or geriatrics.
