We are currently seeking a highly motivated Community Connector to join our team in Enable Ireland Adult Services, Limerick. As a key member of our Strive project team, you will play a vital role in supporting service owners with physical disabilities to transition to new homes in their chosen community.
Your key responsibilities will include:
- Working closely with the Strive Project Team Lead to deliver key outcomes for individuals in areas of independent living and personal development
- Supporting service owners to build a lifestyle and live as part of their chosen community
- Developing and implementing Person Centred Plans to meet the unique needs of each individual
To be successful in this role, you will require:
- Minimum qualification QQI Level 5 Certificate in Social Care or relevant healthcare area
- At least one year's experience working with adults with disabilities
- Proven ability to work as a keyworker and drive Person Centred Plans
- A full clean driving licence with no restrictions
