Job Title: Patient Support Associate
Job Description:

The Patient Support Associate plays a pivotal role in the healthcare system, serving as a central point of contact for cancer patients. Working closely with the clinical nurse specialist team, this individual aims to enhance patient care and communication. The key responsibilities include coordinating remote surveillance, supporting patients on self-managed follow-up pathways, and ensuring personalized stratified follow-up and supported self-management services are in place.


Key Responsibilities:
  • Act as the primary point of contact for patients accessing supported self-managed follow-up pathways
  • Support and coordinate remote surveillance for patients on supported self-managed follow-up pathways
  • Coordinate all aspects of patient aftercare, including surveillance investigations and health and wellbeing events
  • Provide administrative support for meetings and attend as required
  • Ensure accurate attention to detail in own work and work of the team

Requirements:
  • Professional qualifications, experience, or relevant third-level qualification
  • Eligible applicants will be those who have satisfactory experience as a Clerical Officer or obtained specific qualifications
  • Candidates must possess requisite knowledge and ability for proper discharge of office

This is a permanent contract with a full-time schedule. The ideal candidate will possess excellent communication and organizational skills, with previous experience in healthcare administration and direct patient interaction.
