Key Responsibilities:
  • Conduct thorough intake assessments to identify participants' strengths, challenges, and goals.
  • Develop and implement comprehensive rehousing plans, incorporating community resources and partnerships.
  • Provide emotional support, guidance, and advocacy to empower participants to take control of their lives.
  • Cultivate strong relationships with internal teams, community organizations, and external stakeholders to facilitate referrals, collaborations, and service coordination.
Requirements:
  • Master's degree in Social Work, Counseling, or a related field; or equivalent experience.
  • Minimum 3 years of direct experience in case management, crisis intervention, or a related field.
  • Strong understanding of trauma-informed care, child development, and family dynamics.
  • Exceptional communication, interpersonal, and problem-solving skills.
  • Ability to maintain confidentiality and adhere to HIPAA regulations.
