- Deliver programmes within a person-centred model of service delivery by facilitating choices for service users.
- Develop and deliver programmes tailored to meet the needs of service users as identified through Person Centred Planning.
- Facilitate access to community activities and programmes that meet identified needs.
Key responsibilities include:
- Providing key worker support to facilitate the Person Centred Planning process.
- Identifying goals and required supports for individual service users and maintaining accurate records.
- Maintaining records as necessary.
The role will work closely with colleagues to achieve objectives.
Required qualifications and experience include:
- Minimum QQI Level 5 in Social Care or equivalent.
- A third-level graduate in Health & Social Care or equivalent is desirable.
- Full manual driving licence required.
