Job Title: Community Health Care Nurse

We are seeking an experienced and skilled Community Health Care Nurse to join our team. The successful candidate will be responsible for delivering high-quality, person-centered care to service users in their homes and in clinic settings.

Key Responsibilities:
  • To assess, plan, implement, and evaluate care to the highest professional and ethical standards within the model of nursing care practiced in the relevant care setting.
  • To work within Primary Care in the PHN service, as part of a multi-disciplinary team delivering a coordinated approach to client care.
  • Maintain a high standard of nursing care and share responsibility for the management of nursing care and the patients' environment.
  • Deliver care in the client's home and in clinic settings, ensuring professional and ethical standards are upheld.
Required Skills and Qualifications:
  • Registered Nurse with current practicing certificate.
  • Ability to work as part of a multi-disciplinary team and provide holistic, person-centered care.
  • Full driving licence and access to appropriate transport for home visits.
