Are you passionate about delivering high-quality patient care? We are seeking a skilled Clinical Care Coordinator to join our team and make a real difference in the lives of patients with rare lung diseases.
The successful candidate will be responsible for coordinating patient care, working closely with multidisciplinary teams, and providing support to patients and their families.
- Develop and maintain relationships with patients, families, and healthcare professionals
- Provide education and support to patients and their families
- Contribute to the development and implementation of care plans
Key Responsibilities:
- Coordinate patient care and provide support to patients with rare lung diseases
- Work closely with multidisciplinary teams to ensure seamless delivery of patient care
Requirements:
- Bachelor's degree in Nursing or related field
- Minimum 2 years' experience in clinical care coordination or similar role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and organizational skills
