Job Title: Primary Care Nurse SpecialistWe are seeking a skilled and dedicated Nurse Specialist to join our Primary Care team.The successful candidate will work as part of a multidisciplinary team, providing high-quality patient care in a community setting.About the Role:This is an exciting opportunity for a registered nurse to develop their skills and expertise in primary care nursing.You will be responsible for delivering evidence-based practice, promoting health and wellbeing, and improving patient outcomes.Key Responsibilities:
  • Nurse-led Wound Management: Develop and implement effective wound management plans, ensuring patients receive optimal care and support.
  • Diabetes Management: Work collaboratively with healthcare teams to provide comprehensive diabetes care, including education and self-management support.
  • Incontinence Assessment and Management: Conduct thorough assessments, develop personalized plans, and provide ongoing support to patients experiencing incontinence issues.
  • Nursing Assessments: Conduct timely and accurate assessments, identifying patient needs and developing effective care plans.
  • General Nursing Care: Provide high-quality, patient-centered care to individuals with chronic illnesses or disabilities.
  • Oncology Support and End-of-Life Care: Deliver compassionate and empathetic care to patients and their families, addressing their physical, emotional, and spiritual needs.
Requirements:
  • NMBI Registration: Be registered as a Registered General Nurse (RGN) with the Nursing and Midwifery Board of Ireland.
  • Community Nursing Experience: Possess experience working in community nursing settings, with a strong understanding of primary care principles and practices.
  • Acute Experience: Have experience working as a general nurse in acute care settings, with a solid foundation in clinical skills and knowledge.
  • Wound Care Experience: Demonstrate expertise in wound care, including assessment, management, and education.
