Job Title: Programme Facilitator

We are seeking a skilled and compassionate individual to join our team as a Programme Facilitator.

This role is focused on promoting and supporting independent living and community integration by designing and implementing person-centred programmes that match the service user's needs and capabilities.

The ideal candidate will have a strong background in social care, excellent communication skills, and the ability to work collaboratively with individuals and groups.

Key responsibilities include developing and delivering person-centred programmes, being a key worker to service users, identifying goals and supports required by individual service users, and maintaining records as required.

The successful candidate will also support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes.

Requirements for this role include a minimum QQI Level 5 in Social Care or equivalent, good knowledge of New Directions and community-based programmes, and a minimum of one year's experience working in a relevant setting.

As a Programme Facilitator, you will play a vital role in enhancing the lives of people with disabilities and their families.
