Seeking a compassionate and skilled Licensed Practical Nurse to provide medical support to our clients in a safe environment. The ideal candidate will have excellent interpersonal skills and a current state LPN license.

  • Participate in planning and coordinating total patient care in conjunction with the RN and physician's plan of care, following the nursing care plan for each patient.
  • Accurately report and record the patient's condition and care, including signs and symptoms indicative of change.
  • Prepare clinical/progress notes and summaries.
  • Assist patients with activities of daily living and teach appropriate self-care techniques.
  • Provide and maintain a safe environment for patients.
  • Assist physicians and Registered Nurses in complex nursing situations.
  • Follow universal precautions during all patient care activities.
  • Follow emergency procedures in case of catastrophic events, incidents, or significant changes in patient condition.

Qualifications

We are looking for individuals who possess the following qualifications:

  • 1 year of experience as an LPN in a home care setting.
  • Current state license as an LPN.
  • Graduate of an accredited practical nursing program.
  • Proof of current CPR certification.
  • Ability to work a flexible schedule.
  • Excellent interpersonal skills.
  • Valid driver's license and reliable transportation.
  • Willingness to travel locally and work flexible hours.
