Skilled Nursing Care Professional

We are seeking an experienced Skilled Nursing Care Professional to join our team. This is an exciting opportunity for individuals who enjoy working with a complex patient population in short-term rehab and long-term care settings.

  • The ideal candidate will have experience analyzing patient medical history, medication allergies, physical condition, and examination to determine best care for acute and chronic medical conditions.

Responsibilities:

Provide medical care to patients in skilled nursing facilities

Analyze patient data to determine best care options

Order and interpret diagnostic tests to identify and assess patient clinical problems and healthcare needs

  • Prescribe pharmaceuticals and other medications as appropriate for assessed medical conditions
  • Record patient physical findings, formulated plans, and prognosis in the medical record
  • Coordinate with nursing facility care planning teams to develop comprehensive care plans
  • Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care

Requirements:

Masters or Doctoral degree in Advanced Practice Nursing (APRN) or Physician Assistant studies

  • Certified Nationally as a Nurse Practitioner through AANP or ANCC
  • Valid Connecticut license as a Registered Nurse and APRN, and certification as a Nurse Practitioner by an approved certifying body
  • DEA and Ct controlled substance license
  • Experience working with elderly patient populations and electronic health records
