Home Health Care Registered Nurse Job Description

We are seeking a compassionate and skilled Home Health Care Registered Nurse to join our team. As a Registered Nurse, you will provide medical support to our clients in their homes, utilizing your excellent interpersonal skills and ability to work independently.

Job Responsibilities:

  • Provide skilled nursing care to patients on an intermittent basis in their place of residence.
  • Initiate the plan of care and necessary revisions based on patient needs.
  • Make regular evaluations of the patient's nursing needs and make recommendations for changes in the plan of care.
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences as per Agency policy.
  • Coordinate services with other healthcare professionals as needed.
  • Inform personnel of changes in the condition and needs of the patient.
  • Counsel the patient and family/significant others in meeting nursing and related needs.
  • Participate in peer review and performance improvement as assigned.
  • Give total patient care as needed.

Qualifications:

  • Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
  • One year of nursing experience.
  • Kinnser - EMR experience preferred.
  • Home Healthcare experience preferred.
  • Experience in wound vac, PICC line and mediport care, preferred.
  • Pediatric experience, desired.

Working Environment:

  • Works indoors in Agency office and patient homes.
