Home Care Nurse Opportunity

We are seeking a skilled and compassionate Registered Nurse to provide medical support to clients in their homes. As a Home Care Nurse, you will play a vital role in helping patients recover from illness or injury, manage chronic conditions, and maintain their overall health and well-being.

Responsibilities:
  • Administer skilled nursing care to patients on an intermittent basis in their place of residence.
  • Makes initial evaluation visits and regularly reevaluates patient needs to ensure that the highest level of care is provided.
  • Initiates plan of care and necessary revisions based on patient needs and progress.
  • Provides services requiring substantial specialized nursing skills, such as wound care, IV therapy, and medication management.
  • Initiates preventive and rehabilitative nursing procedures to promote patient health and independence.
  • Prepares clinical and progress notes for each patient visit to document treatment and outcomes.
  • Counsels patients and family members on meeting nursing needs and provides education on self-care techniques.
Requirements:
  • Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
  • One year of nursing experience, preferably in home healthcare or a related field.
Working Environment:
  • Works indoors in Agency office and patient homes.
