COMMUNITY SUPPORT WORKER

Job Purpose:

To assist in the delivery of individualized community support to a number of individuals with intellectual, physical, and mental health challenges.

Key Responsibilities:

  • Assist in planning, developing, implementing, monitoring, and reviewing educational, recreational, and occupational programs within the community.
  • Liaise with families, multidisciplinary teams, and community-based personnel to ensure accessible community activities for service users.
  • Document information, update records, and complete necessary paperwork for communication and accountability purposes.

Requirements:

  • Minimum QQI Level 5 in Health and Social Care or equivalent.
  • Willngness to participate in Rehab Care training program and relevant external training opportunities.
  • Experience in the area of supporting individuals with intellectual, physical, and mental health challenges.
