Job Description:

We are seeking an experienced Youth Care Professional to join our team as a Lead Practitioner.

The successful candidate will have a minimum of three years' experience in delivering trauma-informed care to young people aged 13-17.

In this role, you will be responsible for fostering positive attachments through relationship building and providing a therapeutic environment for children and young people.

Key Responsibilities:

  • Care and Placement Plans implementation and recording
  • Promoting rights and responsibilities of each child/young person
  • Ensuring physical, emotional, social, and religious welfare of each young person
  • Encouraging attendance at school, training centres, medical appointments
  • Organising personal documents, arrangements, information, and finance on behalf of the young person
  • Liaising with parents, family, services, and other agencies
  • Attending meetings related to care and development
  • Engaging in age-appropriate play and activities
  • Physical intervention and restraint (following local policies)
  • Preparing meals and undertaking household management
  • Reporting to the Social Care Manager on matters affecting service delivery

Requirements:

  • Minimum three years' experience in Children & Families Services or similar environments
  • Registered in the Social Care Workers Register maintained by CORU or hold a CORU-approved qualification
