Home Care Services Manager

Job Overview
The Home Care Coordinator is responsible for managing and co-ordinating person-centered care and resources in the home to individuals living with dementia. This role requires strong leadership and organizational skills, as well as the ability to work effectively with clients, caregivers, and other stakeholders.
Key Responsibilities
- Manage and coordinate home care services, including supervising caregivers, scheduling shifts, and ensuring adequate staffing levels
- Assess client needs and develop individualized care plans that meet their unique requirements
- Collaborate with healthcare professionals, caregivers, and family members to ensure seamless care delivery and address any concerns or issues that arise
- Maintain accurate records of client information, care plans, and service delivery
- Ensure compliance with regulatory requirements and maintain up-to-date knowledge of industry best practices
Requirements
- Education: Diploma in Health Service Management or equivalent qualification desirable
- Experience: Previous experience working in a caring environment, preferably with individuals with dementia, older adults, or those with learning disabilities
- Skills: Strong leadership and organizational skills, effective communication and interpersonal skills, ability to work independently and as part of a team
