Lead Cancer Care Coordinator

This is a fantastic opportunity to join St. James Hospital as a Lead Cancer Care Coordinator in Systemic Anti-Cancer Treatment (SACT).
The successful candidate will be responsible for coordinating the delivery of high-quality, patient-centered care to individuals receiving systemic anti-cancer therapy in both inpatient and outpatient settings.
You will work closely with the multidisciplinary team to ensure seamless service delivery and contribute to quality improvement initiatives.
- Key Responsibilities:
- Coordinate the development and implementation of individualized care plans
- Ensure timely and effective treatment delivery through collaboration with healthcare professionals
- Provide education and support to patients and their families
- Contribute to quality improvement initiatives
Requirements:
- A minimum of 1 year's post-registration experience in SACT
- Successful completion of a post-registration program in specialist practice
- Demonstrated ability to practice safely and effectively within scope of practice
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
