We are seeking a highly skilled and empathetic individual to join our team as a Social Care Assistant, playing a vital role in delivering high-quality support and services to our clients.
As a key member of our day and residential support team, you will work collaboratively with individuals to identify their needs, develop and implement person-centred plans, and promote their independence and autonomy.
Main Responsibilities- Assess and address the unique needs of each individual, developing and implementing tailored support plans.
- Support individuals with daily living tasks, such as personal care, meal preparation, and household management.
- Collaborate with the social care team and other stakeholders to ensure comprehensive and person-centred support.
- Contribute to the ongoing evaluation and improvement of our services, ensuring they meet the evolving needs of our clients.
- Minimum of a QQI Level 5 Major Award on the QQI Framework - BA in Social Care Studies or equivalent qualification in Community/Health or Social Care.
- Proven experience working with vulnerable adults or adults with intellectual disabilities.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Full driver's licence and availability of own car.
