Job Summary: We are seeking a highly motivated and experienced Rehabilitation Specialist to join our Midlands-based team. In this role, you will work closely with clients, statutory agencies, and community partners to provide comprehensive reintegration and aftercare services. Your primary focus will be on supporting clients through the recovery process, developing individualized care plans, and facilitating group sessions.

Key Responsibilities:

• Deliver reintegration and aftercare services, focusing on substance abuse recovery and relapse prevention.
• Develop and facilitate group sessions, workshops, and one-to-one interventions, tailored to client needs.
• Conduct comprehensive assessments, identify client strengths, and develop individualized care plans.
• Collaborate with statutory agencies, community partners, and family members to ensure seamless service delivery.
• Maintain accurate records, reports, and statistics, ensuring compliance with funding agency requirements.
• Participate in audits, evaluations, and quality assurance initiatives to continuously improve service standards.

Requirements: To succeed in this role, you will need a relevant 3rd-level qualification (Level 7 or equivalent) and a minimum of 2 years' experience in addiction, community development, health & social care, aftercare, and rehabilitation. Your skills should include case management, group facilitation, and key working. A full clean driver's license is essential for this position.
