Independent Living Support Specialist
About the Role:The Independent Living Support Specialist empowers older individuals to live independently and age in place by delivering personalized support and wellbeing plans, and innovative assistive technology. This role ensures tailored, proactive solutions that enhance quality of life and prevent crises.
Main Responsibilities:- Support Coordination: Develop and implement comprehensive support plans to address challenges and enhance the quality of life for older individuals.
- Partnership Building: Establish and maintain strong partnerships with healthcare, housing, social care, and community-based service providers to ensure integrated care and high-quality support delivery.
- Care Assessment: Conduct thorough assessments to identify and address the needs of older individuals, resolving challenges and mitigating risks while regularly monitoring and reviewing care plans to ensure they remain aligned with individual needs.
- Performance Monitoring: Track key performance indicators (KPIs) and strive to achieve SMART targets that maximize the reach and impact of support services for older individuals.
- Advocacy: Proactively advocate for older individuals and identify potential issues through collaborative problem-solving.
- Patient Handling; Manual Handling; Basic First Aid desirable but not essential.
- Minimum 1-2 year experience providing person-centered care and/or support.
- Diploma, Certificate, or Degree in relevant areas of community, social care, or related fields, is desirable but not essential.
- Excellent communication skills (both written and verbal), and the ability to network and cooperate well with individuals, groups, and stakeholders.
- Strong problem-solving and analytical skills, including the ability to adapt your practice to meet the evolving needs of older individuals.
