Homelessness Prevention Advocate

Cpl Healthcare and its partner organisation work together to deliver critical services for individuals facing homelessness in Dublin, Ireland.
This role presents an opportunity to work within a multi-disciplinary/multi-agency service, engaging with families to identify their needs, develop tailored support packages, and implement agreed education programmes.
As a Homelessness Prevention Advocate, you will work collaboratively with stakeholders, including Health Service Executive, Tusla, Probation, and other services, to ensure effective communication and pre-discharge planning.
You will contribute to a case conferencing model, working closely with external professionals and families to ensure effective liaison.
The role requires a deep understanding of child welfare issues, homelessness, and the complexities of working with high need families.
Key responsibilities include working with customers to identify significant needs/behaviours affecting their situation, attending meetings and reviews as required, and implementing policies and procedures.
A minimum of 2 years' experience working with high need families at high risk is essential, utilising an intensive case management and family support model.
Desirable qualifications include a BA in Social Care or equivalent, experience of working with customers who have co-existing conditions, and team working in social care or clinical disciplines.
Excellent communication skills, report writing abilities, and knowledge of children, adolescents, and issues affecting parents are highly valued.
