Job Description

We are seeking a skilled Clinical Nurse Specialist to join our Integrated Care team for Older Persons. As part of our multidisciplinary team, you will provide high-quality care and support to older adults in our community.

The successful candidate will be responsible for assessing patients' needs, developing personalized care plans, and collaborating with healthcare professionals to ensure seamless care delivery.

You will have the opportunity to work closely with our WICOP Waterford and WEXICOP Wexford teams to provide comprehensive care services to older adults in their homes and community settings.

Required Skills and Qualifications
  • A strong background in gerontological nursing and experience working with older adults is essential.
  • Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Able to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
  • Strong assessment and problem-solving skills, with the ability to develop and implement effective care plans.
