Job Title: Community Health Care Professional

We are seeking a skilled Community Health Care Professional to join our team of dedicated healthcare professionals. This is an excellent opportunity for nurses looking to make a positive impact in the community.

About the Role:

The successful candidate will work within Primary Care, providing care and support to clients in their homes and clinics. The work base will be at the Primary Care Centre, where you will have access to onsite training and orientation provided by HSE staff.

Key Responsibilities:
  • Deliver high-quality wound care management in clinic and domiciliary settings
  • Provide diabetes management and incontinence assessment and management services
  • Nursing assessment post hospital discharge or GP referral
  • General nursing care for chronically ill or disabled clients
  • Oncology support and end-of-life care
  • Nursing assessment for home supports
Requirements:
  • Access to own transport required for this post
  • Work authorisation mandatory
  • NMBI registration as an RGN essential
  • Community nursing experience preferred but not essential
