Public Health Nursing Opportunity

This role offers a unique chance for nurses to advance their career in public health nursing, providing essential services to communities.

About the Role:
  • The appointed nurse will work within Primary Care as a community nurse, delivering care in both home and clinic settings.
  • The work base will be the Primary Care Centre, focusing on delivering high-quality patient care.

Responsibilities include wound care management, diabetes management, incontinence assessment and management, nursing assessment post hospital discharge/GP referral, general nursing care for chronically ill or disabled individuals, oncology support and end-of-life care, and assessments for home support services.

Key Responsibilities:

Nursing Skills: This role requires a strong background in nursing skills, including but not limited to: wound care management, diabetes management, incontinence assessment and management, nursing assessment post hospital discharge/GP referral, general nursing care for chronically ill or disabled individuals, oncology support and end-of-life care.

Clinical Expertise: The successful candidate must possess excellent clinical expertise and a strong understanding of community nursing principles.

Communication Skills: Effective communication is critical in this role, requiring the ability to engage with clients, colleagues, and other healthcare professionals to provide exceptional patient care.
