Job Title: Care Services Coordinator

 

Lots of Care offers a range of respite, residential and support services to individuals with intellectual disabilities in a homely and comfortable community setting.

We aim to empower people with the necessary skills to live full and satisfying lives as equal citizens of their local communities.

The successful candidate will have a passion for delivering high-quality person-centred care and will be committed to ongoing professional development.

Key Responsibilities:

  • Developing and maintaining Occupational Therapy (OT) services to ensure high-quality person-centred care
  • Maintaining and tracking key performance indicators (KPIs), including waiting lists, caseloads, multidisciplinary team input etc
  • Developing and maintaining professional standards based on evidence-based practice
  • Creating and maintaining an ethics-based OT service delivery model
  • Generating a quality-driven service supporting service owner health and wellbeing
  • Maximizing service owner active involvement in the development of their care pathway
  • Reviewing OT caseload management to ensure safe and efficient service outcomes
  • Staying updated on new developments, skills and technology
  • Developing, auditing and reviewing policies and procedures in light of changing service needs
  • Coordinating training and education of staff involved in OT service delivery
  • Maintaining, reviewing and reporting on an OT risk register
  • Promoting innovation in future service delivery across OT services
  • Ensuring OT compliance with internal policy and external needs that impact Lots of Care
