About the Role
As a Patient Support Coordinator, you will play a vital part in our mission to reduce the burden of cancer on individuals and their families. Our organization provides a free transport service to referred patients, identified as being in need by our hospital partners. You will be part of a small team of administrators who manage daily bookings from patients, process patient referrals, engage with our partner hospitals, process financial support applications, and encourage patient feedback on service delivery for continuous improvement.

Main Responsibilities
- Receive and collate referral information from our partner hospitals ensuring patient database is up to date.
- Process patient referrals from our partner hospitals, making contact with the patient and taking booking information.
- Process daily patient bookings and input information on CRM (Dynamics) database
- Manage incoming and outgoing patient calls and emails responding to a large volume of incoming calls from an expanding patient base.
- Address logistic issues as they arise, ensuring proper procedures and quality standards are adhered to.
- Offer support to patients signposting them to other supports where appropriate.
- Provide cover within the Driver Support team if and when required.

Evaluation and Continuous Improvement
- Administer patient feedback surveys when required and collate the findings.
- Ensure continuous service improvement, through patient feedback and signposting to other services and supports.
- Proactively identify opportunities for improvement in our service delivery or operations.

Stakeholder Engagement and Relationship Building
- Build and maintain relationships with existing internal and external stakeholders and develop new relationships as required.
- Work closely with other teams within the Society to provide a full support service to all referred patients.
- Build effective relationships across the Society and work in collaboration with Services Team and wider Irish Cancer Society colleagues to contribute to the successful delivery of the organisational strategy.
- Keep up to date with legislation, policies and procedures, and undertake appropriate training.

Requirements
- A degree or professional qualification in social work, community development, or related field.
- Additional relevant work experience will be considered in lieu of a formal education qualification.
- A minimum of 3 years relevant work experience in service delivery or community support.
- The Irish Cancer Society measures experience by the quality of the experience gained, rather than number of years worked.
- An understanding of patients' needs and an empathy for patients who find themselves in difficult situations.
- Ability to work as part of a team and on your own initiative.
